Completing this form will serve as your written notice of cancellation, as required by your membership agreement. Your membership will be canceled 14 days from the date you submit the completed form, unless you specify a different date that is at least 14 days away. If you have a scheduled payment during this period, it will be processed as planned and prorated based on the contract end date.

Please note that any unused sessions will expire on the last day your membership is active and will not be transferred to any future memberships.

We implement these policies to manage our schedule effectively and minimize disruptions. Short-notice cancellations can create open spots that take time to fill, as we need to provide advanced notice for new openings. We appreciate your understanding.

If you choose to reactivate your membership in the future, please be aware that the membership rates in effect at that time will apply, as rates are subject to change.
